By submitting personal information, including, but not limited to, name, address, date of birth, and medical information to Travelex Insurance Services Canada Inc. (“Travelex”), Zurich Insurance Company Ltd (Canadian Branch), and World Travel Protection Canada Inc. (collectively, “Zurich”), their affiliates, and authorized representatives respecting individuals insured or covered by this policy, I acknowledge and confirm that I have consented to and, if applicable, I have obtained, and am retaining the consent of the named individuals in this application for travel insurance included above, to the collection, storage, use and disclosure of their personal information for the purposes of securing and administering insurance coverage(s). I understand that Personal information may be processed and stored by Zurich, its affiliates, and authorized representatives in both domestic and foreign jurisdictions.

The above-named applicant (s) may refuse to consent or withdraw their consent to the collection, storage, use or disclosure of personal information; however, the refusal to provide consent may result in Travelex being unable to offer and administer insurance coverage or prevent Zurich from being able to pay claim benefits.

Travelex is committed to protecting the privacy and confidentiality of information provided. Your file is secured in our offices or those of our administrator or agent. Consent to Conduct Business Electronically Terms & Conditions

By agreeing to receive Communications via electronic delivery you agree to the following Terms and Conditions:

You have indicated your desire to conduct business electronically with Travelex. This includes electronically receiving and signing Communications such as: documents relating to your insurance and other transactions (current and future); documents relating to a claim (current or future); and all billing documents. In any event, Travelex may continue to send certain insurance related documents via non-electronic delivery.

This Consent to Conduct Business Electronically applies to all Communications that may be given to you, and in some cases signed and returned to us, as part of an insurance transaction. By agreeing to these Terms and Conditions, you are agreeing that you have the authority to receive these documents electronically on behalf of all insureds under your policy.

The words “Travelex”, "we," "us," and "our" means Travelex Insurance Services Canada Inc., its affiliates, subsidiaries, parent companies, or any unaffiliated company for which Travelex provides policyholder administrative services. The words "you" and "your" mean you, the individual(s) who desires to transact business or process a claim electronically, as well as any named or additional insured under your policy.

Method of Providing Communications to You in Electronic Form. All Communications that we provide to you in electronic form will be provided via the e-mail address you provide below and will be in a downloadable form that you must keep in your records. It is your responsibility to ensure that you retain a copy of any Communication Travelex delivers to you electronically.

How to Withdraw Consent. You may un-enroll from electronic communications by contacting Travelex by phone at: 1-844-799-0335, by sending a letter to Travelex, 901 King Street West, Ste 300, Toronto, ON M5V 3H5 or by email to privacy@travelexinsurance.ca. At our option, we may treat an invalid email address or the subsequent malfunction of a previously valid address as a withdrawal of your consent to receive electronic Communications. We will not impose any fee to process the withdrawal of your consent to transact business electronically. If, however a discount is given now or in the future for conducting business electronically, a withdrawal of consent could result in the removal of any such discount. A withdrawal of your consent to transact business electronically will be effective only after we have had reasonable time to process your request.

How to Update Your Records. It is your responsibility to provide us with true, accurate and complete e-mail address, contact, and other information related to these Terms and Conditions and your insurance policies, and to promptly update or notify us of any changes in this information. You can update your information (such as your e-mail address) by contacting Travelex via phone at 1-844-799-0335, by sending a letter to Travelex, 901 King Street West, Ste 300, Toronto, ON M5V 3H5 or by email to privacy@travelexinsurance.ca. If we are unable to consistently deliver email notifications successfully, you will be automatically unenrolled from receiving Communications electronically.
